Admission to a graduate-level program requires a previous degree in a related discipline with a minimum B+ average. Acceptable disciplines include chemistry and biochemistry. Some schools also require some form of advanced laboratory experience in an organic chemistry research environment (eg. summer research project, industry experience, etc.).
Admission to a PhD program can either be directly from an undergraduate degree (in addition to already having extensive research experience, and being published in a peer-reviewed journal), or after acquiring a Master's-level degree.
